Prostate cancer and male health prevention

Prostate cancer is the most common neoplasm in the male population, accounting for nearly 20% of all cancers affecting Italian men and 26% of solid tumors in Western countries. Every year in Italy, there are about 41,100 new diagnoses, making prevention and early detection essential. The presence of germline mutations in DNA repair genes, particularly in the BRCA1 and BRCA2 genes, exposes men to a higher risk of developing certain cancers, including prostate cancer. Identifying men with pathogenic variants in the BRCA genes and subjecting them to personalized screening could help in the early diagnosis of potential prostate cancer, with significant implications for therapy and survival.

HPV, or Human Papillomavirus, is very common, typically transmitted through sexual contact, and affects both males and females. In most cases, patients who contract the infection overcome it asymptomatically; however, in some cases, depending on the specific type of HPV, the following may occur:

  • Benign lesions on the skin and mucous membranes (e.g., warts on the genitals, face, hands, and feet)
  • Condylomas (growths) or papillomas on the genital and oral mucous membranes.

In more severe cases, the virus settles in the cells, creating a kind of chronic inflammatory stimulus that over time causes cellular damage leading to tumor degeneration.

For men, prevention is essential through HPV analysis using samples from the urethral swab, balanopreputial area, semen, anal swab, and oropharyngeal swab to rule out the presence of an infection that could develop into cancerous forms in the penis, anus, or in extragenital sites such as the oral cavity, pharynx, and larynx.

However, prevention for male health must also address sexually transmitted infections (STIs). STIs, if not diagnosed and treated in a timely manner, can impair the proper functioning of organs and systems and can lead to conditions ranging from inflammation to more severe issues like infertility and cancer.
Other STIs are caused by agents such as:

– Chlamydia trachomatis
– Neisseria Gonorrhoeae
– Mycoplasma genitalium
– Mycoplasma hominis
– Ureaplasma urealyticum
– Ureaplasma parvum
– Trichomonas vaginalis
– Gardnerella vaginalis

These pathogens can cause, if not properly eradicated: sterility/infertility, pregnancy complications (e.g., preterm birth, miscarriage, stillbirth), neonatal infections (e.g., eyes, lungs), and the development of tumors.

In this regard, performing urethral or balanopreputial swabs or analyzing semen is essential for detection through highly sensitive and reliable molecular techniques.
